Who's eligible for a GP Management Plan?

Anyone with a chronic medical condition lasting 12+ months that needs ongoing multidisciplinary care. Your GP will assess at a longer appointment.

Are the allied health visits really free?

Yes — under the Team Care Arrangement, you can access up to 5 bulk-billed allied health sessions per calendar year at our clinic (dietitian, physiotherapist, podiatrist).

Can I have multiple care plans?

You usually have one GPMP at a time, but it can be reviewed and adjusted as your needs change.

What if I don't have one chronic disease but several issues?

Many patients have multiple conditions — that's exactly when a GPMP works best. It coordinates everything in one plan.

How long does it take to write a care plan?

The initial appointment is 30–45 minutes. Reviews are 15–30 minutes. We use this time to ensure the plan is really yours.
